Hackley Community Care Center receives a number of questions each day. Below is a complied list of some of the more common questions and their answers by subject. If you have a question concerning medical terminology, please see our glossary of terms.

Billing Answers:
1. If I have questions concerning my bill, who do I contact?
    You can address all billing questions in an email to: Accounts Receivable or you can call our Accounts Receivable Department direct at (231) 733-6607. This information is also found under “Contact Information” on our website. Top
2. My bill says "balance forward." What does that mean?
     This term appears when a previous billing statement was mailed to you but was not paid in full. The balance outstanding is brought forward to the next billing statement. Top
3. What is a “guarantor”?
     A guarantor is the person who is financially responsible for the bill. This might be the patient, the parent of the patient, a spouse, etc. Top
4. How do I know what my insurance will cover?
     Before visiting any doctor, you should check with your insurance company to see what your financial obligation might be. Top
5. What if I don’t have insurance?
     Payment options may be available to you. Your income will be assessed at your first appointment to see if you are eligible for our Sliding Fee Scale program. This program is based on your income and family size; please have this information available at your visit. The Sliding Fee Scale offers you a reduced fee for medical, dental, and pharmacy services. Hackley Community Care has also made special arrangements to provide limited optometry, x-ray, lab, and many other hospital services at a reduced fee for your convenience. Space is limited, so contact our outreach coordinator at (231) 733-6614 for further information about your options. Top 
6. Why am I required to show my insurance card at every visit?
     Insurance plan benefits, eligibility and billing addresses change frequently, so some carriers require us to verify your insurance each time you visit. Checking your card only takes a minute, and it can ensure accurate and timely billing. Top
7. How and where do I pay my bill?
     Payments are always accepted in the office during business hours. See “Contact Information” for office hours. We accept Visa, MasterCard, debit cards and of course cash. We don’t accept personal checks. We will accept a money order mailed to the address on your statement. Top
8. I cannot afford my bill. Can I obtain financial assistance?
     Contact our Outreach Coordinator at (231) 733-6614 for information on qualifying for special assistance. If you are already on our Sliding Fee Program or don’t qualify for our Sliding Fee Program, please contact our billing department at (231) 733-6607 for possible payment plan arrangements. Top

1. What should I bring with me at the time of my visit?
     Please bring:
o        your insurance card,
o        personal ID,
o        co-pay or sliding fee scale payment and any past due owed,
o        a list of your current prescriptions or better still your actual prescription bottles Top
 
2. What is a FQHC?
     FQHC stands for Federally Qualified Health Center. The FQHC benefit under Medicare was added effective October 1, 1991 when section 1861(aa) of the Social Security Act was amended by Section 4161 of the Omnibus Budget Reconciliation Act of 1990. FQHC’s are “safety net” providers whose main purpose is to enhance the provision of primary care services in underserved urban and rural communities. 3. Is Hackley Community Care Center part of Mercy Health partners?
     Hackley Community Care Center is independent from Mercy Health partners. Our roots go back to Hackley Hospital opening us up in 1992 and all though we formally disaffiliated from Hackley Hospital in 2000, we retain a positive working relationship with the Hospital. Top
4. What programs and services does Hackley Community Care Center offer?
     We offer Medical, Dental, OB, Maternal Infant Health, Behavioral Health, Patient Assistance Program, Pharmacy, Grandparents Raising Grandchildren Program, Breast & Cervical Cancer Program, School Based Health Program, and Chronic Disease Self-Management Programs. Top
